DRUG POLICY. Malone Staffing and their clients realize the importance of a safe working environment. As a result, we are sometimes requested by our clients to have employees tested for illegal drugs, substances, and alcohol. A positive test or refusal to submit to a urine or blood/breath alcohol screen may result in disciplinary action up to and including termination of employment.
NO SHOW - NO CALL. It is expected that if you accept a position, but cannot go, that you will notify Malone Staffing ASAP. This is the responsible thing to do and hopefully gives us the time to find a replacement for the client. One No Show without good reason will result in automatic inactivation.
TERMINATION NOTICE. A two-day notice of termination is required. If you quit without a two-day verbal/written notice or walk off the job, then your pay for that week will be reduced to minimum wage. Exceptions may be made if there is a valid reason for leaving the job site without notification.
ASSIGNMENTS. When your assignment ends you must call the office and report your availability within 48 hours of your termination notification. Until you are placed on another assignment you are to contact Malone Staffing once a week to check in for work. Failure to do so will be indication that you are no longer available for work and have voluntarily quit Malone Staffing.
EMPLOYEE RECORDS. If you change your name, address or telephone number you must report the change immediately to Malone Staffing. Failure to do so will be indication that you are no longer available for work and have voluntarily quit Malone Staffing.
HARASSMENT. Malone Staffing will not tolerate any form of sexual harassment by an employee or any other individual who conducts business with Malone Staffing. If you feel like you are experiencing any type of harassment, you must notify a Malone Staffing Manager immediately.
BACKGROUND CHECK. Malone Staffing may contact and obtain information about you from any or all of your references, former employers, credit agencies, educational institutions, person, and law enforcement agencies. Consequently, you release Malone Staffing, its employees and all other persons and corporations from all liability and responsibility arising out of such inquiry or the response to such inquiry.

Further, I understand and agree that if Malone Staffing hires me, my employment relationship with Malone Staffing is at will. This means that just as I have the right to end my employment with Malone Staffing at any time, for any reason or no reason, with or without notice, Malone Staffing has the right to end my employment with Malone Staffing at any time, for any reason or no reason, without notice. Further, I agree to be responsible regarding the duties required on each assignment. I agree to be on time, observe regulations, be well groomed, and suitably dressed, perform to the best of my ability, and complete the assignment. I understand that Malone Staffing will pay me at an hourly rate specified when accepting each assignment. All pay rates are confidential and I agree not to disclose or discuss the information. I will not accept permanent employment from any company (customer) of Malone Staffing to whom I have been assigned within 90 days of my last assignment to the same company without the documented consent of Malone Staffing. Malone Staffing is authorized to make a thorough investigation of my work history by contacting former employers and personal references. I verify that all information provided by me on this and all Malone Staffing forms is correct. I will hold Malone Staffing harmless from any claims including, but not limited to, personal injury or illness as a result of my providing false information on this application. I understand that failure to fulfill any part of the above agreement may result in termination.
